We are excited that you wish to join our faith community.  Please complete this on-line registration form and either click the submit button at the bottom to send it by e-mail or print out your completed form and mail it to St. Philip the Apostle Parish, 1223 W. Holtz Avenue in Addison, IL  60101. 

Offertory envelopes are provided by an outside firm and are printed every other month.  Until you receive them, you may simply put your name on a plain envelope to be put in the Sunday offertory, and your account will be credited.  You may also choose to contribute your offertory directly from your checking account.  If you would like to take advantage of this option, please complete this electronic funds transfer form and mail it to our parish business office at the address listed above.
